Every student — new and returning — must register each year.
It is very important for families to register or re-register as early as possible.
After July 1, the district will use those registration numbers to determine staffing at each of our schools.
RETURNING STUDENTS:
If you have not registered by August 1, 2024, you will need to show proof of residency, your child won’t be included on a class list, and your student will not be signed up for Transportation at the start of the school year until you register.
Below are the steps to enroll your child for the 2024-25 school year in District 15.

1. Determine Eligibility
Are you a resident of District 15?
Per Illinois law and Board of Education policy, only students who are legal residents of District 15 may attend school in the district. Generally, a student’s residency (proof of living situation) is the same as the person who has legal custody of the student. The person registering the student must submit (1) proof of legal custody and (2) residency (proof of living situation).
Proof of Residency (Proof of Living Situation)
Proof of residency (proof of living situation) is required for all NEW students (kindergarten and students new to the district), families flagged by the third-party verification system, and all families with month-to-month and yearly LEASES.

Is your child old enough to begin kindergarten?
To begin kindergarten in Illinois, a child should be 5 years old on or before September 1 of the kindergarten year.
If your child will not be 5 on or before September 1, District 15 does offer early childhood and pre-school options.
2. Complete Online Registration
NEW FAMILIES: START A NEW REGISTRATION APPLICATION
For families new to District 15—click this link to start a new registration application to enroll your child in a District 15 school.
EXISTING FAMILIES: SIGN IN TO CAMPUS PARENT
Existing parents use Campus Parent to register their child(ren) for the upcoming school year, including any children new to the District. You will need your parent username and password to sign in to your Campus Parent account.
GATHER THE FOLLOWING INFORMATION

Proof of residency (proof of living situation) documents - Proof of residency (living situation) is required for all incoming kindergarten students, all families with leasing agreements, and families with students new to the District. You have the ability to upload required Proof of Residency (living situation) documents in Infinite Campus. If you are unable to upload these documents, you will need to bring them in person to the school office to provide your Proof of Residency (living situation).
- Household information (address and phone numbers)
- Parent/guardian information (work and cell phone numbers, email addresses)
- Student emergency contacts (names, home phone, cell phones, work phones, emails)
- Student's birth certificate (you'll need to enter their full legal name as it appears on their birth certificate)
- Student's prior school attendance information, if applicable (school name, city, state, country, phone)
- Name of primary health and primary dental providers (names, phone numbers)
- Student health history - Students entering CCSD15 for the first time are required to submit a State of IL Certificate of Child Health Examination form with parent/guardian and doctor signatures prior to attending school. There are a number of questions on the online registration form regarding your child's health history and medical conditions, so having this form in hand may be helpful. Student health history will need to be re-entered each year, it does not roll over from previous years.
Note: if you are unable to complete the entire application at one time, don't worry, you can come back to it later to complete it. Remember to write down your application number, you will need this to return to the application. You will be able to print or save a copy of the application for your records before submitting the application. Once the application is SUBMITTED, you will no longer be able to make changes. Please contact your school for any necessary corrections. If needed, you will receive an email from the school letting you know what documents/information will be needed to complete your registration.

3. Provide Required Documentation to the School
Depending on your situation, you may need to drop off required documentation to the school.
- Birth Certificate or Passport
- Home Language Survey
- Proof of Residency (Living Situation)

Birth Certificate or Passport
All students who are newly registering with the district are required to provide a Birth Certificate or Passport (original with seal). Official birth certificates are issued by the county clerk in the county in which the child was born. Hospital certificates are not acceptable. A certified copy of the child’s birth certificate must be provided within 30 days of enrollment.
Please bring a certified copy of your child’s birth certificate (with seal) or other government document verifying date of birth. An official certificate of every birth should be on file in the locality where the event occurred. These records are filed permanently in a State vital statistics office or in a city, county, or other local office.
To obtain a certified copy of a birth certificate, write or go to the vital statistics office in the State or area where the event occurred, or visit the National Center for Health Statistics (www.cdc.gov/nchs/w2w/index.htm) for information on where to obtain an official birth certificate.
Home Language Survey
A Home Language Survey is administered to all students newly registering in the district to help us determine if your child is eligible for language support services. Families will be provided with the Home Language Survey in their spoken language at their home school.
Proof of Residency (Living Situation)
Proof of residency (proof of living situation) in District 15 is required for all incoming Early Childhood students, Kindergarten students, students new to the District, existing families with a lease, and families that are flagged by our third-party residency (living situation) verification system to complete the registration process.
District 15 requires all families with NEW students (kindergarten and students new to the district), families flagged by the third-party verification system, and all families with month-to-month and yearly LEASES to prove residency each year.
For the 2024-25 school year, in the case of a lease, please note that if the copy of the lease provided expires before September 6, 2024, the family will need to provide an UPDATED lease prior to the start of the school year to remain enrolled for the 2024-2025 school year. The lease needs to show that the family lives in District 15's boundaries at the start of the 2024-2025 school year.
Required Health, Immunization, Dental, and Eye Exams
Parents are required to provide health, immunization, dental, and eye exams for students entering preschool or kindergarten, sixth-grade, and students new to the District. It is recommended that families keep a copy of their child’s health examination and immunizations for their records.
Health Exam
Health exam dated within one year of school entry is required. The Certificate of Child Health Examination form is available online and in the school office. Please submit early to assure your child has met the health requirements and any health issues are planned for in advance of school entry. Submit completed health exams to the home school before the start of school (September 5, 2024). We accept proof of appointment for those unable to complete the examination prior to the first day of school.

Immunization Records
Immunization records. We accept vaccine documents at any time upon registration. Please submit early to assure your child has met the State of IL vaccine requirements. The month/day/year and correct number of doses at the correct intervals must be verified by a health care provider. This is due before school starts.

Objections Based on Religious Grounds
Parents/guardians who object to examination(s) or immunization(s) on religious grounds are not required to submit their child or wards to either, and must provide a completed Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form signed by the health care provider. Per School Board Policy 7:100, Health, Eye, and Dental Examinations, Immunizations, and Exclusion of Students—Students who are not in compliance with IL student health requirements may be excluded from school.

Half-Day Kindergarten
Two-Way Dual Language Program
District 15's Two-Way Spanish Dual Language Program is a two-way language immersion where students are taught literacy and curriculum in both Spanish and English beginning in kindergarten and continuing through the completion of fifth grade. The goal of the Dual Language program is to develop bilingualism, biliteracy, academic achievement, and cross-cultural competencies for students. The program is available to incoming kindergarten students who are fully registered with their home school for the 2024-25 school year. It is housed at Jane Addams, Kimball Hill, and Winston Campus Elementary Schools.

Applications to enroll your child in the program are due by March 15, 2024.
2024-25 Application for Incoming Kindergarten Two-Way Spanish Dual Language Program
Families who apply for the Dual Language Program will be notified of their placement by the end of the day of April 22, 2024. If the number of families interested in Spanish Dual Language exceeds the amount of space available, based on enrollment, a random selection process will be conducted.
One-Way Spanish Dual Language Program (for Native Spanish Speakers)
One-Way Spanish Dual Language Program (for native Spanish speakers) is offered to students who are native Spanish speakers and qualify for language support services. Eligibility is determined via an English proficiency screening.
Students in kindergarten receive 80 percent of their instruction in Spanish and 20 percent of their instruction in English by a certified instructor. For each subsequent year, the percentage of instruction in Spanish decreases by 10 percent. By third grade, instruction in the two languages is balanced at 50 percent Spanish and 50 percent English.

5. Pay Student Fees
Pay Student Fees
Student fees for the 2024-25 school year:
- Grades K-5 (book/material/technology fees): $180/student/year
- Grades 6-8 (book/material/technology fees): $195/student/year
Student fees are due July 1, 2024, for the 2024-25 school year.
How to pay Student Fees
An invoice is not mailed home. Student fees may be paid by cash, check, credit card or e-check.
- Cash payments
Cash payment should be made by the parent in the office of the child's attendance school, where a payment receipt will be issued.
- Check payments
Checks should be made payable to "CCSD15," and can be mailed to your child's school or the District Office, Community Consolidated School District 15, 580 N. 1st Bank Drive, Palatine IL 60067-8110. Please note the student's name, amount paid, and parent's name on the payment envelope. Returned check fee charge is $25 per check.
- Credit card or e-check payments
Credit card or e-check payments may be submitted through Infinite Campus: https://ic.ccsd15.net/campus/portal/parents/palatine.jsp. You will need your Campus Parent portal email address and a valid credit card number. If you need assistance with your Campus Parent Portal email address, please call your child’s school for assistance. Parents who do not have Internet access may come to their child's school to process their credit card payments. A small convenience fee is added for credit card processing.

Student Fee Waivers
Students who are homeless, as defined in the McKinney-Vento Homeless Assistance Act, or who qualify for the free or reduced-price meals program, will have student fees & fines waived.

New parents — Create an Infinite Campus/Campus Parent portal account. After registering your child online, and the school has accepted your registration, new families will receive information in July on how to set up their Campus Parent portal account. Campus Parent allows parents to view their child's course schedule, assignments, attendance, grades, fees that are due, and more.
Campus Parent is designed to provide real-time access to student information. The easy-to-use design displays what is currently happening in the classroom so you can understand, monitor, and participate in the educational process.

- If your child will attend Whiteley, Hunting Ridge, Marion Jordan, Pleasant Hill, or Willow Bend Elementary; or Thomas Jefferson or Plum Grove Middle School
- If your child will attend Central Road, Sanborn, Jane Addams, Kimball Hill, Lake Louise, Lincoln, Paddock, Virginia Lake, or Winston Campus Elementary; Carl Sandburg, Sundling, or Winston Campus Middle School; or Conyers Learning Academy
If your child will attend Whiteley, Hunting Ridge, Marion Jordan, Pleasant Hill, or Willow Bend Elementary; or Thomas Jefferson or Plum Grove Middle School
If your child will attend Frank C. Whiteley, Hunting Ridge, Marion Jordan, Pleasant Hill, or Willow Bend Elementary, or Thomas Jefferson or Plum Grove Middle School, and your household meets federal guidelines, your child may be eligible for free or reduced-price meal services under the National School Lunch and School Breakfast programs.
To apply for free or reduced-price meals, fill out the Free & Reduced-Price Application Form. Parents/guardians may also apply online through SchoolCafé. A NEW APPLICATION MUST BE FILLED OUT EVERY SCHOOL YEAR.
- Income eligibility information and FAQs for the Free & Reduced-Price Meal Program can be found on the application. Applications can be submitted at any time during the school year. Households should answer all applicable questions on the form. Applications that do not contain all required information cannot be processed and approved.
- Once approved, the free or reduced-price meals will be automatically discounted in the student's meal account. Reduced-price meals are $0.40 for lunch and $0.30 for breakfast. Students who qualify for free or reduced-price meals are entitled to both breakfast (if available at the school) and lunch. If a student wishes to purchase a second meal or any a la carte items, the student must have cash available or money deposited in their account in advance.
If your child will attend Central Road, Sanborn, Jane Addams, Kimball Hill, Lake Louise, Lincoln, Paddock, Virginia Lake, or Winston Campus Elementary; Carl Sandburg, Sundling, or Winston Campus Middle School; or Conyers Learning Academy
If your child will attend Central Road, Gray M. Sanborn, Jane Addams, Kimball Hill, Lake Louise, Lincoln, Stuart R. Paddock, Virginia Lake, or Winston Campus Elementary; Carl Sandburg, Walter R. Sundling, or Winston Campus Middle School; or Conyers Learning Academy, free meals are part of the federally funded Community Eligibility Provision (CEP). Students at these schools are offered a free breakfast and lunch daily.

Students’ eligibility for free or reduced-price meals is confidential. If you receive a letter in mid-July to early August that you have been approved by direct certification, you DO NOT need to fill out an application for free or reduced-price meal benefits. Otherwise, A NEW APPLICATION MUST BE FILLED OUT EVERY SCHOOL YEAR. Meal benefits expire in October if you do not complete an application for the current school year.

District 15 offers breakfast and lunch for a fee. Visit Nutrition Services for information about meals offered, menus, and more!
For your convenience, parents/guardians can pay for school meals and monitor student account balances, transactions, and request low balance notifications online using SchoolCafé:
- Stay informed about your student's lunch account activity.
- Be notified on your phone when your student's account is low.
- Make secure payments with Visa, MasterCard, or Discover Card. There is a 5% service fee to use SchoolCafé.
- Store your credit/debit card for quick and easy repeat use.
- Schedule deposits to occur in any frequency you choose.
SchoolCafé allows for access from the web or any mobile device.
- The SchoolCafé website uses the most up-to-date security available. The online payment plan is an enhancement to the district’s existing pre-pay meal program. Cash or checks will still be accepted.
- The SchoolCafé apps for both iOS and Android are now available for download. They're free to install and they make your favorite features even easier to use. Download the SchoolCafé app today on the AppStore or Google Play.

Asthma Action Plan
An increased awareness of asthma and allergies is needed to keep students safe at school. The Health Services staff maintains a list of students with asthma to share with staff on a need-to-know basis. Asthma information and recommended actions for responding to emergency events, including emergency medication, are provided to staff. Illinois law requires students with asthma to provide an Asthma Action Plan that guides staff response.
Diabetes Care Plan
A Diabetes Care Plan will be developed and implemented for students diagnosed with diabetes, per the Care of Students with Diabetes Act. When parents/guardians submit a medical Diabetes Medical Management Plan to the school district, the plan will then be incorporated into an IEP or Section 504 plan, by a certified school nurse with protocol for implementation and staff training.
Food Allergy & Anaphylaxis Emergency Care Plan
If your child has life-threatening anaphylactic allergies (including food allergies), please notify your school nurse and print the Food Allergy and Anaphylaxis Emergency Care Plan for completion by your physician, and submit it to your school's health nurse. The nurse maintains a list of students who have anaphylactic allergies to share with staff on a need-to-know basis and can assist you with the necessary documents needed for school.
Seizure Action Plan
For students with epilepsy, a Seizure Action Plan is needed to provide appropriate care while the student is at school. All school employees receive training in the basics of seizure recognition and first aid and appropriate emergency protocols.
Health Plan - Medication Authorization Forms
CCSD15 believes that prescription and nonprescription medication should be administered in the home. However, if it is in the best educational and health interest of the child to take prescribed medication during the school day, then a Medication Authorization Form must be completed. This pertains to all medications (prescription, over the counter, and emergency). Medication must be brought to the school by the parent/guardian in the original container. Transportation - Download My Ride K-12 (GPS Bus Locator)
My Ride K-12 is the new bus GPS system which can be used by families to help track their child's bus. The My Ride K-12 app will enable you to check the bus stop times and bus stop locations for your student(s), and receive updates directly from the Transportation Department regarding any changes in daily routes. To use it, parents/guardians need to download the mobile app from the App Store or get it on Google Play.
Transportation - Paid Rider Transportation/Busing
In most cases, students who live more than 1.5 miles from their home school will be provided free bus service through CCSD15. State funding is not provided for students living within 1.5 miles of their schools. These students may ride the bus for a fee provided that space is available and a route already exists. (No stops will be made less than one-half mile from the school building.)
An Application for Paid Transportation will be available on the Transportation webpage for download on/after September 5, 2024. Please submit a separate application for each student for whom you choose paid transportation.
Due to the bus driver shortage, paid ridership continues to be limited and only offered if space is available on an existing bus route. Applications will be accepted, but payment will not be accepted until a parent receives notification that their child has been assigned to a bus route. Bus availability will be evaluated after October 1.
Paid Rider Transportation Fees for 2024-25 are:
- One child - $370
- Two children - $470
- Three or more children - $520
Transportation - Voluntary Opt Out of Bus Service
If your child is eligible for free bus service, but you do not intend to use the bus or only need transportation before or after school (not both ways), please fill out the Transportation Opt-Out form to indicate for which children and for which bus routes (before and/or after school) bus transportation is not needed.
If a student lives 1.5 miles or more away from school, students are entitled to free bus transportation. Unless a family notifies us that they do not want transportation, their children are placed on a bus route. Some families do not want to use transportation and choose to transport their children to and from school themselves. If we remove students from bus routes that are NOT going to use district transportation, we can reduce bus route times by eliminating the bus stopping at that assigned bus stop. At present, the bus stops at all assigned stops on the route and waits 3 minutes for the students to board the bus. If there are no students at that stop, the bus still waits for 3 minutes before moving onto the next stop.
If your child(ren) is assigned to a bus route and you wish to continue using district transportation, you do not need to do anything. The OPT OUT form is ONLY for families that do not need all or partial D15 bus transportation.
7. Arrange Before- and/or After-School Care
If you need before and/or after-school childcare, don't delay in setting that up!
Before- and After-School Care Providers
Before- and after-school care programs are available at District 15’s schools. These programs are operated independently of the school district by the respective park districts or organizations. Visit their websites for general information, pricing, enrollment, and FAQs.
- Hoffman Estates Park District STAR Program offers before- and after-school care for students who attend Frank C. Whiteley.
- Palatine Park District C.A.R.E. Program offers before- and after-school care for students who attend Gray M Sanborn, Jane Addams, Lake Louise, Lincoln, Marion Jordan, Stuart R. Paddock, Virginia Lake, and Winston Campus Elementary.
- Rolling Meadows Park District R.E.C. Program offers before- and after-school care for students who attend Central Road and Kimball Hill.
- Right At School Program offers an additional after-school care option at Hunting Ridge, Pleasant Hill, and Willow Bend.
There are several private community care providers in District 15 boundaries that offer before- and/or after-school care. Click here for information about daycare center transportation to/from school.

Kindergarten Two-Way Spanish Dual Language Applications are due. All families applying must also have completed Kindergarten registration with the district to be considered. Applications submitted after this date (March 15) will be placed on a waiting list.
Being considered for the Gifted program? If your student is in grades 2, 3, or 4 and is scheduled to take the CogAT exam to be considered for the Gifted Program, you will be notified of your student’s eligibility status by March 15, 2024. All elementary Gifted programming is provided at Stuart R. Paddock School.

- All students who have not completed their registration by August 1 will be disenrolled from District 15. Families that do not complete the registration by August 1 should contact their school to re-enroll their child(ren).

Bus Stops and Pickup/Drop Off Times
Parents/guardians will receive an email before school starts with their child's bus stops and pick up/drop off times.For students who receive free bus service (students who live 1.5 miles or more away from school), but do not need or intend to use the bus, or only need transportation one-way (to or from school), please fill out the Transportation Opt-Out form so we can remove your child(ren) from that route. Indicate which children and which routes you do NOT need transportation for.
My Ride K-12 is the new bus GPS system which can be used by families to help track their child's bus. The My Ride K-12 app will enable you to check the bus stop times and bus stop locations for your student(s), and receive updates directly from the Transportation Department regarding any changes in daily routes. To use it, parents/guardians need to download the mobile app from the App Store or get it on Google Play.
Reporting an Absence/Sick Child
When do I keep my child home from school?
- Fever: Keep your child home if they have a fever (temperature of 100°F or greater). Keep your child home for a period of 24 hours after the fever has passed, without fever-reducing medication.
- Vomiting or diarrhea: Keep your child home for 24 hours after the symptoms have stopped.
- Undiagnosed rash
- Communicable diseases (such as strep throat, influenza, chicken pox, measles, COVID, etc.): Keep your child home for a period of 24 hours of antibiotic treatment or after the contagious period for the illness is over. This time frame may be longer for more serious communicable diseases.
- Pink eye (conjunctivitis)
- Head lice infestation (live head lice)
Please report illness symptoms completely on the attendance line for your school.

For families that are NEW to District 15: use the ONLINE REGISTRATION KIOSK to start a new registration application to enroll your child in a District 15 school.
For families who have children already enrolled in District 15: use the CAMPUS PARENT link to register your child(ren) for the upcoming school year, including any children new to the District. You will need your parent username and password to sign in to your Campus Parent account. If you forgot your password or username, click on the "Forgot" links.
What is my "home school"? Which school will my child attend?
A "home school" is the school assignment based on your home address. Please keep in mind that based on programmatic placements, your child may not attend their "home school." See the Bus Tiers and Program Placements tab below for more information.

Can I request that my child attend a different school in the district?
Under special circumstances, and on a very limited basis, parents/guardians may request an Intra-District Transfer of their child(ren) from their assigned home school to another intra-district school. Requests will only be considered upon completing the requested application and providing supporting documentation within the announced timeline. Requests are handled by the District 15 Intra-District Transfer Committee. Each spring, District 15 will announce the dates and deadline to submit requests for consideration for the upcoming school year.
My child attends a local daycare before and/or after school. Is there transportation available to/from the daycare?
Only children who are residents of District 15 may attend District 15 schools from a daycare center from within the district. The daycare must be within the home school boundaries of the student. Please review the list of local daycare centers and if School District 15 provides transportation from the daycare to the school, or if the daycare center provides transportation to the school.
- If your child’s daycare center is located within the school attendance boundary associated with your home address, your child may be eligible for district-provided transportation if the daycare center is located 1.5 or more miles away from the student’s home school or if hazardous crossings exist as defined by the State of Illinois.
- If your child’s daycare center is not located within the school attendance boundary associated with your home address, and you would like the District to consider allowing your child to attend another school where the daycare center is located, you will need to request an Intra-District Transfer on behalf of your child.
The Intra-District Transfer Committee makes a final determination on the status of each transfer request received. In the event that the Intra-District Transfer Committee approves an intra-district transfer request, parents are responsible for all costs associated with providing transportation for their child to and from school. Requests are approved on a year-to-year basis.
PLEASE NOTE: Space for pay riders is limited and only offered if space permits.

District 15 offers a Spanish Dual Language Program at three schools for incoming kindergarten students.
How can I stay informed about school and district news?
District 15 uses the the Finalsite (formerly Blackboard) mass notification system to communicate with parents and staff via phone, text, email, and the D15 mobile app. Once your child is enrolled in the district, all parents are automatically subscribed and will receive regular newsletters from their child's principal and district.
